An alert has been issued for a missing West Hempstead man, according to Nassau County Police.
Thomas O'Dwyer, 75, of West Hempstead, was reported missing Christmas Day.
O'Dwyer is 5'8," 160 pounds with white hair and gray eyes.
He is considered to be a vulnerable adult with dementia, a heart condition and kidney failure.
O'Dwyer was last in contact with family members via phone on Dec. 24. He is believed to be driving a 2014 blue Honda Accord with New York

He may appear lost or confused and may be in need of medical attention.
